How We Calculate

Augmentin dosing in pediatrics is based on the amoxicillin component. Standard dosing at 25 mg/kg/day is appropriate for mild infections. For acute otitis media and bacterial sinusitis, the AAP recommends high-dose amoxicillin/clavulanate at 45 mg/kg/day to overcome intermediate-resistant S. pneumoniae. For documented resistant organisms, 90 mg/kg/day using the ES-600 formulation is recommended.

The ES (extra strength) 600 mg/5 mL formulation has a lower clavulanate-to-amoxicillin ratio, allowing higher amoxicillin doses without exceeding the clavulanate threshold that causes diarrhea.
